But other medical bodies point to the considerably lower risks of pouches compared with smoking. The disagreement highlights a deeper question in European tobacco policy: is the objective to reduce smoking-related disease, or nicotine use itself?<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The German Cancer Research Center (DKFZ) <a href=\"https:\/\/www.dkfz.de\/fileadmin\/user_upload\/Krebspraevention\/PB_2026_Verkaufsverbot-f%C3%BCr-Nikotinbeutel-beibehalten.pdf\">recently called on German policymakers<\/a> to keep the country&#8217;s existing sales ban on nicotine pouches in place.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Around 6 percent of German teenagers and 19 percent of young adults have already tried the products, the institute notes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;The sales ban on nicotine pouches in Germany serves youth protection and preventive health protection,&#8221; Ute Mons of DKFZ said <a href=\"https:\/\/www.dkfz.de\/aktuelles\/pressemitteilungen\/detail\/jugendschutz-vor-lobbyinteresse-dkfz-fordert-erhalt-des-verbots-von-nikotinbeuteln\">in a statement<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;It makes the products less visible and prevents manufacturers from marketing them as attractive, trendy lifestyle products to a young audience.&#8221;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Notably, however, DKFZ does not argue that nicotine pouches cause cancer.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Pouches contain no tobacco leaf and involve no combustion. That distinction matters because cigarette smoke contains thousands of compounds, including dozens known to cause cancer, generated largely through the process of burning tobacco.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The immediate German debate is instead about legal classification. Nicotine pouches are currently regulated as an unauthorised novel food, effectively keeping them off store shelves.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Manufacturers want them regulated as tobacco-related products instead, opening the possibility of a legal market. DKFZ is urging the government to resist what it calls &#8220;Lobbydruck&#8221; \u2013 lobby pressure \u2013 and retain the existing ban.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">For Patrik Str\u00f6mer, Secretary General of the Association of Swedish Snus Manufacturers, that distinction raises an important question about the role of organisations focused on cancer prevention.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;There is a perfectly legitimate debate to be had about nicotine dependence, youth use and how these products should be marketed,\u201d he explains.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cBut that is different from a debate about cancer risk. When a cancer institute argues for banning a product without claiming that it causes cancer, we should at least ask what public-health outcome it is trying to achieve.&#8221;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Different definitions of harm reduction<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The debate is not confined to Germany.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The <a href=\"https:\/\/pharmaceutical-journal.com\/article\/feature\/everything-you-need-to-know-about-oral-nicotine-patches\">UK&#8217;s Royal College of Physicians<\/a> has reviewed the evidence on alternative nicotine products and placed oral nicotine pouches considerably lower on the risk spectrum than smoking, behind only medicinal nicotine replacement products such as gum and patches.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That comparison introduces an important distinction.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Nicotine replacement therapy deliberately maintains nicotine consumption while helping smokers move away from cigarettes. Public-health authorities generally accept that continued nicotine use can be justified when it substantially reduces exposure to the harms caused by smoking.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The question is whether the same principle should apply to non-medical nicotine products.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Sweden&#8217;s Hj\u00e4rt-Lungfonden <a href=\"https:\/\/www.hjart-lungfonden.se\/halsa\/riskfaktorer\/tobak\/vitt-snus\/forskning\/\">has taken a different approach<\/a>. Its eight-point action plan calls for &#8220;ett nikotinfritt Sverige&#8221; \u2013 a nicotine-free Sweden \u2013 by 2040. It argues that nicotine pouches are not an effective smoking cessation aid and may instead act as a gateway to tobacco smoking.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Cancerfonden, Sweden&#8217;s cancer charity, has similarly <a href=\"https:\/\/www.cancerfonden.se\/om-oss\/about-us\/swedens-road-to-reduced-tobacco-smoking\">challenged the idea<\/a> that nicotine pouches should be regarded as smoking cessation products.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In Poland, the Polish Society of Oncology and the Polish Society of Cardiology <a href=\"https:\/\/www.pto.med.pl\/wspolne-stanowisko-polskiego-towarzystwa-onkologicznego-oraz-polskiego-towarzystwa-kardiologicznego\">have gone further<\/a>, jointly stating that e-cigarettes, heated tobacco and nicotine pouches &#8220;do not meet the definitional criteria of harm reduction strategy.&#8221;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The disagreement therefore concerns more than the relative toxicology of different products. It also reflects competing definitions of harm reduction.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Under one interpretation, harm reduction means reducing the disease and mortality associated with smoking, even if some people continue using nicotine. Under another, reducing or ultimately eliminating nicotine consumption is itself an important public-health objective.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What counts as harm reduction?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Other medical organisations have reached different conclusions.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Czech Society for Addictive Diseases, whose members specialise in treating addiction, assessed the <a href=\"https:\/\/snncls.cz\/wp\/wp-content\/2022\/03\/SNN-CLS-JEP-nikotinove-sacky.pdf\">harm from nicotine pouches as &#8220;very low&#8221;<\/a> compared with other tobacco and nicotine products.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In an official position paper commissioned by the Czech Ministry of Health, the organisation concluded that pouches &#8220;represent a suitable harm reduction alternative&#8221; to smoking cigarettes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Royal College of Physicians has <a href=\"https:\/\/pharmaceutical-journal.com\/article\/feature\/everything-you-need-to-know-about-oral-nicotine-patches\">similarly emphasised<\/a> the importance of distinguishing between nicotine products according to their relative risks.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Str\u00f6mer argues that this difference in perspective should be made more explicit in the policy debate.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;If the goal is a nicotine-free society, organisations should say so clearly and make the case for it,\u201d he says.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cBut if the goal is reducing cancer, cardiovascular disease and lung disease caused by smoking, then relative risk has to matter. Those are two different objectives, and they can lead to very different policy choices.&#8221;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">David Nutt, professor of neuropsychopharmacology at Imperial College London, made a similar argument in a <a href=\"https:\/\/clivebates.com\/papers-and-letters\/cop11experts\/\">statement published ahead of the WHO&#8217;s COP11 conference<\/a>: &#8220;There are no other areas of public health where just demanding abstinence or trying to enforce abstinence via prohibition is seen as a credible strategy.&#8221;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">A question for Brussels<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Germany&#8217;s current legal situation is unlikely to remain isolated from the wider European debate.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The EU&#8217;s forthcoming revision of the <a href=\"https:\/\/snusforumet.se\/en\/tag\/tpd\/\">Tobacco Products Directive<\/a>, TPD3, is expected to address the regulation of newer nicotine products, potentially including nicotine pouches.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Decisions taken in Brussels could therefore influence how these products are classified and regulated across the bloc.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That makes the disagreement between DKFZ and organisations such as the Czech Society for Addictive Diseases more than an academic dispute.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It raises a fundamental policy question: what outcome should regulation be designed to achieve?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Reducing youth nicotine uptake and preventing dependence are legitimate public-health objectives. So are reducing smoking prevalence, cancer deaths, cardiovascular disease and lung disease. But those objectives do not necessarily point toward identical policies.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The debate over nicotine pouches therefore ultimately requires policymakers to distinguish between two related but different ambitions: reducing nicotine use and reducing the health damage caused by smoking.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">As Str\u00f6mer puts it:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;The important thing is to be clear about what problem we are trying to solve,\u201d he says.&nbsp;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cIf nicotine use itself is the problem, that is one policy debate. If smoking-related disease is the problem, then we need to compare products according to the risks they actually pose. We shouldn&#8217;t pretend those are the same question. And we know that a ban creates an illegal market.
