With over a billion smokers still lighting up, some argue it\u2019s time to shift focus from ideology to impact. \u201cIf you want fewer smokers, look at Sweden,\u201d says Patrik Str\u00f6mer. \u201cWe\u2019ve done it \u2013 not with bans, but with alternatives that actually work.\u201d<\/strong>\u00a0<\/p>\n\n\n\n<p>With the lowest smoking rates in Europe and correspondingly low levels of tobacco-related disease, the Swedish experience is a living case study in effective harm reduction. But will anyone at COP11 be willing to learn from it?&nbsp;<\/p>\n\n\n\n<p>Patrik Str\u00f6mer, Secretary General of the <a href=\"https:\/\/snustillverkarforeningen.se\/\">Association of Swedish Snus Manufacturers<\/a>, is both hopeful and sceptical.\u00a0\u00a0<\/p>\n\n\n\n<p>\u201cI always hope more people will understand the vast difference between smoking and using smoke-free nicotine products,\u201d he tells Snusforumet.&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cBut I also know there are many who simply don\u2019t want that to be true.\u201d&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Sweden\u2019s success \u2013 and WHO\u2019s blind spot&nbsp;<\/h2>\n\n\n\n<p>Despite having the <a href=\"https:\/\/snusforumet.se\/en\/sweden-tobacco-tax-travesty-punish-snus-users-and-keep-cigarettes-cheap\/\">cheapest cigarettes in Scandinavia<\/a> and a <a href=\"https:\/\/www.tobaccocontrolscale.org\/wp-content\/uploads\/2022\/12\/TCS-Report-2021-Interactive-V4.pdf\">Tobacco Control Scale (TCS) rating that places it 21st<\/a> in the EU, Sweden boasts the lowest smoking rates on the continent. Str\u00f6mer points out that this directly challenges WHO\u2019s standard recommendations, which include increasing excise taxes on cigarettes.\u00a0\u00a0<\/p>\n\n\n\n<p>\u201cThat recommendation simply isn\u2019t true,\u201d he says.&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cSweden proves that there are factors stronger than traditional methods like tax hikes.\u201d&nbsp;<\/p>\n\n\n\n<p>At the core of this contradiction lies a broader issue: the WHO\u2019s reluctance to engage with real-world data.&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cIt would be hugely beneficial if the WHO chose to study Sweden and make recommendations based on reality rather than ideology,\u201d says Str\u00f6mer.&nbsp;<\/p>\n\n\n\n<p>\u201cAs long as the \u2018quit or die\u2019 mindset persists, smoking will continue to be a societal problem.\u201d&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">FCTC \u2013 a treaty past its prime?&nbsp;<\/h2>\n\n\n\n<p>This year\u2019s COP comes at a symbolic moment \u2013 20 years since the adoption of the FCTC. But instead of celebrating progress, Str\u00f6mer believes the framework now risks doing more harm than good, arguing the FCTC has \u201coutlived its usefulness\u201d.&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cIts refusal to acknowledge that people will choose safer alternatives if available is the biggest threat to public health,\u201d he explains.&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cIf they truly want fewer smokers, just do what Sweden has done. For them, the stakes of COP11 are real and urgent.&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cWill they get access to accurate information?\u201d Str\u00f6mer asks.&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cWill they be allowed to choose safer products than cigarettes? There\u2019s a lot at risk here.\u201d&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The harm reduction disconnect&nbsp;<\/h2>\n\n\n\n<p>Str\u00f6mer is sharply critical of how harm reduction is often dismissed by FCTC delegates and anti-tobacco activists as merely an \u201cindustry narrative.\u201d He calls this line of thinking not only unscientific but dangerous.&nbsp;<\/p>\n\n\n\n<p>\u201cIt\u2019s propaganda,\u201d he says bluntly.&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cIt\u2019s like calling gravity an industry narrative or that the second law of thermodynamics is a conspiracy for energy companies. If your theory can\u2019t explain what\u2019s happening in reality, you need a new theory.\u201d&nbsp;<\/p>\n\n\n\n<p>Most Swedes, he adds, understand that <a href=\"https:\/\/snusforumet.se\/fakta\/the-swedish-experience\/\">snus has played a central role in reducing smoking<\/a>, while many researchers, politicians, and activists refuse to acknowledge the evidence.\u00a0\u00a0<\/p>\n\n\n\n<p>&#8220;Instead, they invent new ideas that don\u2019t match reality. It\u2019s dishonest \u2013 and harmful,\u201d he says.&nbsp;<\/p>\n\n\n\n<p>He believes it\u2019s possible to criticise smoking and its harmful effects while also concluding that harm reduction works.\u00a0\u00a0<\/p>\n\n\n\n<p>\u201cBut that requires being open to reconsidering your worldview when confronted with facts,\u201d Str\u00f6mer adds.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Hope for COP11 in Geneva?\u00a0<\/h2>\n\n\n\n<p>When asked what he would say to COP11 delegates, Str\u00f6mer doesn\u2019t hesitate: \u201cPlease, look at what works. Let go of activism against the industry and study reality.\u201d&nbsp;<\/p>\n\n\n\n<p>He points out that WHO embraces harm reduction for drug policy, but curiously not for nicotine or alcohol.&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cIn medicine, it\u2019s understood that every treatment has risks. The key is weighing those risks against the benefits,\u201d he explains.&nbsp;<\/p>\n\n\n\n<p>\u201cNowhere is this more obvious than with tobacco: the benefits of quitting smoking are enormous, and the downsides of switching to smoke-free products are comparatively small.\u201d&nbsp;<\/p>\n\n\n\n<p>Yet he worries that ideological rigidity at WHO will once again block progress, and that the EU will follow suit.&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cIf WHO sticks to an outdated line, and the European Commission then uses that as the basis for future regulation, <a href=\"https:\/\/snusforumet.se\/en\/cop11-leaked-eu-doc-puts-future-of-nicotine-pouches-in-doubt\/\">the Swedish success story could be undone,<\/a>\u201d Str\u00f6mer warns.\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Beyond tobacco: Europe&#8217;s credibility at stake&nbsp;<\/h2>\n\n\n\n<p>The implications extend beyond public health.&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cEurope is the continent with <a href=\"https:\/\/www.politico.eu\/article\/europe-world-leader-smoking-rates\/\">the highest share of smokers<\/a>,\u201d says Str\u00f6mer.\u00a0\u00a0<\/p>\n\n\n\n<p>\u201cThat shows policy has failed\u2014except in Sweden, where daily smoking is down to around five percent. If Europe wants to stay relevant, it needs to be willing to revise its approach.\u201d&nbsp;<\/p>\n\n\n\n<p>Str\u00f6mer concludes with a challenge to the policymakers in Geneva: \u201cDo you really want to see fewer smokers? Then study what works.
